Grade 8 Middle Years Survey

Dear Grade 8 Parents/Guardians,


We are reaching out to inform you that Notre Dame Regional Secondary, in collaboration with the University of British Columbia’s Human Early Learning Partnership (HELP), is inviting students in Grade 8 to participate in the Middle Years Development Instrument (MDI) survey. This survey focuses on social and emotional development, health, school experiences, and well-being. Please see the attached notice for further information about the survey, contact information if needed, as well as information if you would like to withdraw your child from participating in the survey.


Key Points:

  • The survey will take place during class time in January/February.

  • Participation is voluntary, and students may skip questions or withdraw at any time.

  • Parents/guardians can request to withdraw their child by returning the attached form within two weeks or contacting the MDI project team at mdi@help.ubc.ca.

  • The survey is confidential, and no individual information will be shared with the school or added to student records.


If you would like to withdraw your child from participating in this survey please complete the form at the bottom of the notice and return it to Vice-Principal Mr. Tagulao in the office.
